Identical twins Elliot and Beverly Mantle (both played by Irons) are famous gynecologists in Toronto. Elliot is the more aggressive and outgoing of the two, who makes the public speeches, is successful with women, and when he tires of them hands women off to his shyer, research-oriented brother. When a famous actress, Claire Niveau (Bujold) comes to their clinic for infertility problems, Beverly falls in love with her and refuses to share his feelings and experiences with his brother anymore. When Claire realizes the two have been playing her off, she leaves, and Beverly descends into a drug habit, also having some ghoulish surgical tools designed for him by an artist. Unable to break his emotional dependence on Elliot, and craving Claire, Beverly and the twins' relationship move toward the breaking point. Cowritten and directed by Canadian horror filmmaker David Cronenberg, this 1988 film is sophisticated and extremely uncomfortable fare. Irons does an outstanding job portraying and distinguishing the two brothers.

Beverly and Elliot Mantle are famous gynecologists and identical twins, Beverly dedicating himself on practice and research while Elliot, more confident, gives lectures. Elliot is helping Beverly meet women and, one day, he organizes a rendezvous between Beverly and one of their famous patients, Geneviève Bujold, an actress. At first reluctant in front of the peculiar situation, Beverly soon falls in love with Bujold and doesn't want to share his thoughts and sexual experiences with Elliot. As Bujold must leave Canada for a while in order to shoot a TV mini-serie, a depressive Beverly develops a drug problem. Eliot tries to cure him but Beverly becomes insane, he even performs a surgical operation with special tools created specially for him by an artist. Eliot knows that if Beverly dies, he will, like the second siamese twin, follow her to his death.
